Create a performance review template

Sets up a fair, consistent format for feedback and goal setting.

Lower risk: public info only — A blank template contains no employee information.

Please note: This site is only a guide to help people understand. Consult your IT, security and/or privacy department for clarification.

Before you start, gather

List the real [KEY COMPETENCIES] from your job descriptions or organizational values. Write them as role skills, never from a named person's file. Afterward, compare the template with your handbook and review cycle.

Starter prompt

Create a performance review template for staff at [ORGANIZATION NAME], a small nonprofit with limited budget for raises. Make it work for people who wear many hats and work with volunteers. Include a self-reflection section, recognition of mission impact and non-pay ways to reward growth, manager feedback, rating scale for [KEY COMPETENCIES], goals for next year, growth and training wishes, and space for comments. Encourage specific examples, and keep it to two pages. Do not state employment-law requirements or review rules as fact; write HR CHECK instead of guessing.

Paste the prompt into your AI tool first, then replace the placeholders with your own details. Don't use real names of clients or donors.

Open an AI tool

You are leaving this guide. Copy the prompt first, then open a tool. The link goes to the website of the company named on each link, in a new tab. That company's terms and privacy policy apply there. Paste the prompt in, then replace the placeholders with your own details. Don't enter real names or confidential details.

Check the result

Review it with HR or leadership and check it is consistent with your employment policies.

What it might get wrong

A rating scale tied to raises you cannot offer. Competencies borrowed from a for-profit template. A claim about how often reviews must happen under the law.
