Draft survey questions for program participants

Produces clear, unbiased questions that people can answer quickly.

Lower risk: public info only — Write the questions only; don't paste in any participant answers that include names.

Please note: This site is only a guide to help people understand. Consult your IT, security and/or privacy department for clarification.

Before you start, gather

Set [WHAT YOU WANT TO LEARN] from your evaluation plan or funder reporting questions. Name [PROGRAM NAME] as participants know it. Do not include any participant's answers.

Starter prompt

Write a short survey of 10 questions for participants of [PROGRAM NAME]. We want to learn [WHAT YOU WANT TO LEARN]. Use simple words, avoid leading questions, mix rating scales and open questions, and keep it to about 5 minutes. Include a one-sentence intro explaining the survey is voluntary. Do not ask about services or sessions I did not mention; where a question needs a detail I did not give, write MISSING INFO instead of guessing.

Paste the prompt into your AI tool first, then replace the placeholders with your own details. Don't use real names of clients or donors.

Open an AI tool

You are leaving this guide. Copy the prompt first, then open a tool. The link goes to the website of the company named on each link, in a new tab. That company's terms and privacy policy apply there. Paste the prompt in, then replace the placeholders with your own details. Don't enter real names or confidential details.

Check the result

Pilot with a few people to catch confusing wording, and check the questions with any funder reporting requirements.

What it might get wrong

Questions that nudge toward praise. Items asking two things at once. Questions about program features you don't offer. A 5 minute length that 10 mixed questions often exceed.
